| Bridges and Pathways Institute Inc is a not-for-profit organisation established in the context of the Australian primary care service reforms to focus on solutions. Its purpose is to improve the health-related outcomes and reduce the burden of chronic illnesses on individuals, their families and the Australian community through: |

- Effective partnerships with Government, Professional Health Bodies, Universities and Consumer Groups
- Improved information and communication at all levels of the community
- Improved access to timely and appropriate continuous care
- Educational activities to support informed decision making and self management
- Systemic advocacy and mediation to improve health management
- Prioritise health outcome focussed research

| The work of the Institute is unique in that our projects are directed towards low priority complex chronic illnesses, focussing on multidisciplinary, shared care management. |

| We are using cost effective epidemiological, disease risk management, education and empowerment frameworks, and applying them in Australian practice settings. |
| Organisational Structure: |
| The Management Executive liaises with people who are supportive of our values. We contract work out through a national team of consultants, specialists, lay experts, working groups, project staff and dedicated volunteers. |

| For people with chronic illnesses the Institute aims to work collaboratively to: |

- Reframe these illnesses for prevention, early intervention and health risk management
- Prioritise health outcomes and health related quality of life
- Foster effective multidisciplinary service delivery systems
- Encourage consumers to have a greater role in decision making
- Facilitate shared care through empowerment, skills sharing and development
- Facilitate monitoring and evaluation of individual care plans and services
